Welcome to the Executive Wellness Blueprint Program and Website!

Our mission is to help others optimize their health and teach them how to be able to take charge of their own wellness.. This may or

may not include virtual one-to-one or group coaching sessions, general health and wellness-related strategy or advice, and

interpretation of certain lab results.

These Terms set forth your rights and obligations as someone who is accessing Executive Wellness Blueprint programs, content,

and services. By interacting with this website; creating a profile; or downloading, creating, viewing, and/or purchasing any products,

services, or content, you indicate that you have read and understood this Agreement and you will be bound by its Terms.

Definitions

A. “Executive Wellness Blueprint” is registered as Wellness Blueprint, LLC (“Executive Wellness Blueprint”), a Washington State

Limited Liability Company. “We,” and “Executive Wellness Blueprint” may be used interchangeably on this site and on products and

services offered on this site, and are used to indicate the company, owners, parent company, and/or agents of the company,

Wellness Blueprint, LLC (also referred to as the “Company”).

B. “Executive Wellness Blueprint” or “Wellness Blueprint” and the associated logo are trademarks, whether registered or

unregistered, owned by the Company. All associated content is copyrighted content owned by the Company.

C. “Parties” mean Executive Wellness Blueprint and You. Executive Wellness Blueprint and You are each a “Party.”

D. “Terms” mean and refer to the Terms of Use set forth herein.

E. “You” and “Your” means any person interacting with Executive Wellness Blueprint website, purchasing products or services on

Executive Wellness Blueprint, creating an Executive Wellness Blueprint profile, and accessing Executive Wellness Blueprint content.

Please refer to the Executive Wellness Blueprint’s Privacy Policy (found here) which explains how we collect, use, and disclose

information that pertains to your privacy. When you access or use Executive Wellness Blueprint, you signify your agreement to this

EXECUTIVE WELLNESS BLUEPRINT WEBSITE – WHAT CAN YOU DO HERE?

By continuing to access this site, you may be able to sign up for an account; access virtual materials; access online resources;

schedule consultations; sign up for virtual classes or programs; order labs; make payments; fill out forms; download materials like

ebooks, articles, or health and wellness plans; and access a ton of useful and really amazing online content about health and

wellness.

Any information or advice given by the Company on the Websites is not meant to take the place of advice of licensed healthcare

professionals including a general practitioner who is hired by you to provide you with specialized and tailored medical care. Unless

otherwise specifically noted, this website contains general advice and guidelines. If You are under the care of a healthcare

professional or currently use prescription medications, you should discuss any changes with your Doctor.

As of the date of this agreement, the Company’s coaches draw upon extensive personal experience with their own health and

wellness journeys and experience working as licensed professionals in the state(s) in which they live. However, just because

someone is a licensed professional or has a background in a certain field, does NOT make them YOUR licensed professional. They

are acting in the capacity of a coach – not your medical provider.

We can provide guidance and strategies, but we cannot do the work for you! We cannot and do not guarantee any sort of specific

result or outcome. You are in charge of your health and we are here to support you as you take charge of your wellness journey.

These are the general commitments and understandings that we expect:

1. You understand that life, your health, wellness, and this journey are all ultimately up to you. The Company and Your Coach is

here to support you, guide you, listen to you, and provide you with strategies and tools; however, no one else can implement

changes for you. If you do not make the changes, then you cannot expect the results that come from change!

● You need to show up for classes and appointments, if applicable.

● You need to make the time to follow-through.

2. You understand that every person and body is different. Do not compare yourself and your journey to someone else’s. The

Company is not guaranteeing or promising any particular result or outcome and the Company cannot promise or guarantee any

result or particular outcome. The Company might tell you stories or show you examples of others and their journeys, but your

journey is unique.

3. You understand that you are not required to do anything that you are uncomfortable doing. The Company will suggest activities,

strategies, and plans for you. You are always invited to say “no.”

4. The Company may refer you to other practitioners – these other practitioners are their own businesses, with their own business

practices and contracts and requirements. You are always welcome to choose your own practitioners.

5. Lastly, You understand that the Company is not providing you with medical, financial, legal, or psychiatric advice or services. If

you need advice in those areas, you should seek a professional licensed in those specific areas where you need help.

REGISTRATION; RULES FOR USER CONDUCT AND USE OF THE SERVICE

You need to be at least 18 years old or have parental/guardian consent to register for, pay for, and use our services or our paid

memberships.

If you are a user who signs up for a class or service, you may create a personalized account which includes a unique username and

a password to access your information and to receive messages from the Company. You agree to notify us immediately of any

unauthorized use of your password and/or account. The Company will not be responsible for any liabilities, losses, or damages

arising out of the unauthorized use of your member name, password and/or account, if you are using one.

EXECUTIVE WELLNESS BLUEPRINT WEBSITE – WHAT CAN YOU NOT DO HERE?

Your permission to use the Site is conditioned upon the following Restrictions. Executive Wellness Blueprint expects you to respect

us and our content. You agree that you will not under any circumstances:

● use the service for any unlawful or unethical purpose or for the promotion of illegal activities;

● use our content without permission;

● provide false or inaccurate information when purchasing any Services;

● interfere or attempt to interfere with the proper functioning of the website or our business;

● make any automated use of the system, or take any action that we deem to impose or to potentially impose an

unreasonable or disproportionately large load on our servers or network infrastructure;

● bypass any robot exclusion headers or other measures we take to restrict access to our site, content, or proprietary

information or use any software, technology, or device to scrape, spider, or crawl the Site, app, or harvest or manipulate

data;

● publish, make derivative works of, or link to our content without Executive Wellness Blueprint’s explicit permission to do

so;

● sharing or selling your login or user information, when applicable; or

● share false or misleading information about Executive Wellness Blueprint.

CONTENT DISCLAIMER

Opinions, advice, statements, offers, or other information or content made available for your use are from the direct experience of

Executive Wellness Blueprint and Executive Wellness Blueprint’s contributing authors. Executive Wellness Blueprint, its affiliates,

and its customers are not guaranteeing any specific result. Your results can vary depending on your body, your health, your age,

your ability, your implementation, your lifestyle, and any number of different factors in your life. Executive Wellness Blueprint strives

to provide impactful advice that can be adapted for use by a wide array of individuals, but there is no substitute for expert,

personalized advice. Executive Wellness Blueprint, the customers mentioned on the Executive Wellness Blueprint website, and the

Executive Wellness Blueprint contributors have experienced the stories and results mentioned and we believe you can create

meaningful and positive changes in your life BUT we can’t guarantee any specific result and we are NOT guaranteeing any specific

result.

We’re also not guaranteeing or endorsing any specific item, person, company, diet, health plan, or activity (unless we specifically

state otherwise).

We are not your Doctor, Nurse, Licensed Dietitian, Licensed Physical Therapist, Licensed Nutritionist, Psychologist, Licensed

Counselor, Naturopath, Physical Therapist, Functional Medicine Practitioner, and/or any other licensed, certified, or registered

professional!

While helping other customers, offering our services, and/or going through our own journeys, we may have made use of various

third party services. Executive Wellness Blueprint may provide you with convenient links to these third party website(s) (“Third Party

Sites”) as well as links to content or applications (“Third Party Services” and “Third Party Content”). These links are provided as a

courtesy to You. If we receive compensation for posting these links, we will let you know as required. However, once you leave

Executive Wellness Blueprint or otherwise access any Third Party Sites, Third Party Services, and Third Party Content or their

promotions, Executive Wellness Blueprint has no control over and no liability for the materials, information, goods or services

available on these Third Party Sites, Third Party Services or Third Party Content. Such Third Party Sites, Third Party Services, and

Third Party Content are not investigated, or monitored for appropriateness, or completeness by Executive Wellness Blueprint on any

regular basis, and Executive Wellness Blueprint is not responsible for them, even if you accessed them through the Site. Inclusion

of, linking to or permitting the use or installation of any Third Party Site, Third Party Services, or Third Party Content does not mean

we are in any responsible for them or the results that you may get using them, it’s just an interesting site or service that Executive

Content, you do so at your own risk and you should be aware that our terms and policies no longer govern. You should review the

If Executive Wellness Blueprint is receiving any financial gains from linking to or referring to a particular site or service, we will be

sure to let you know by labeling the content as affiliate or advertising.

If you are a copyright owner or an agent thereof and believe, in good faith, that any materials on this site infringe upon your

copyrights, you may submit a notification pursuant to the Digital Millennium Copyright Act (see 17 U.S.C 512) (“DMCA”) by sending

the following information in writing to the Company’s designated support email with the subject: “Executive Wellness Blueprint

1. The date of your notification;

2. A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is

allegedly infringed;

3. A description of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online

site are covered by a single notification, a representative list of such works at that site;

4. A description of the material that is claimed to be infringing or to be the subject of infringing activity and information

sufficient to enable us to locate such work;

5. Information reasonably sufficient to permit the service provider to contact you, such as an address, telephone number,

and/or email address;

6. A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the

LICENSE GRANT

By submitting any reviews, anecdotes, case studies, images, videos, or other content to Executive Wellness Blueprint, you

expressly grant, and you represent and warrant that you have a right to grant, to Executive Wellness Blueprint a royalty-free,

sublicensable, transferable, perpetual, irrevocable, non-exclusive, worldwide license to use, reproduce, modify, publish, list

information regarding, edit, translate, distribute, publicly perform, publicly display, and make derivative works of all such Content.

Executive Wellness Blueprint will ask your permission before publishing any identifying information with the Content.

INTELLECTUAL PROPERTY

You acknowledge and agree that we and our licensors retain ownership of all intellectual property rights of any kind related to this

Site, including applicable copyrights on written, audio, video, or produced materials; trademarks; trade dress; domain name rights;

and any content provided on the website, and other proprietary rights, whether identified as proprietary or not, whether implied or

expressly stated. Other product and company names that are mentioned on the Service may be trademarks of their respective

owners. We reserve all rights that are not expressly granted to You or Others under this Agreement. Your use of or interaction with

this Site does NOT grant you any license or permission to use, copy, make derivative works of, or repost any of the proprietary

information on this site, even with attribution, without express, written permission from Executive Wellness Blueprint.

We’re glad you like our stuff. We’re glad you found our stuff useful and think others may find it useful. However, that does not give

you the right to provide it to others or copy it without our permission.

USER CONSENT TO RECEIVE COMMUNICATIONS IN ELECTRONIC FORM

For contractual purposes, you: (a) consent to receive communications from us in an electronic form via the email address you have

submitted; and (b) agree that all Terms of Use, agreements, notices, disclosures, and other communications that we provide to you

electronically satisfy any legal requirement that such communications would satisfy if it were in writing. The foregoing does not affect

your non-waivable rights.

We may also use your email address to send you other messages, including information about the Site or the Service and special

offers. You may opt out of such email by changing your account settings, using the “Unsubscribe” link in the message, or by sending

an email to [email address].

INDEMNITY

You agree to protect, defend, indemnify and hold harmless Executive Wellness Blueprint, its officers, directors, employees,

owner(s), and parent company(ies) and assigns from and against all claims, demands, and causes of action of every kind and

character without limit arising out of Your conduct. Your indemnity obligation includes, but is not limited to, any third party claim

against Executive Wellness Blueprint for liability for payments for, damages caused by, or other liability relating to, You.

NO WARRANTY; NO LEADS

Life is a do-it-yourself adventure. Executive Wellness Blueprint does not and cannot promise, guarantee or warrant any success,

income, or result. Additionally, EXECUTIVE WELLNESS BLUEPRINT’S WEBSITES AND SERVICES ARE PROVIDED “AS IS”

WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING WITHOUT LIMITATION IMPLIED

WARRANTIES OF TITLE, MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND NONINFRINGEMENT.

EXECUTIVE WELLNESS BLUEPRINT MAKES NO REPRESENTATION OR WARRANTY AS TO THE ACCURACY, RELIABILITY,

TIMELINESS OR COMPLETENESS OF ANY MATERIAL ON OR ACCESSIBLE THROUGH ANY EXECUTIVE WELLNESS

BLUEPRINT WEBSITE OR SERVICE. ANY RELIANCE ON OR USE OF SUCH MATERIALS SHALL BE AT YOUR SOLE RISK.

EXECUTIVE WELLNESS BLUEPRINT MAKES NO REPRESENTATION OR WARRANTY (A) THAT ANY EXECUTIVE WELLNESS

BLUEPRINT WEBSITE OR SERVICE WILL BE AVAILABLE ON A TIMELY BASIS, OR THAT ACCESS TO ANY EXECUTIVE

WELLNESS BLUEPRINT WEBSITE OR SERVICE WILL BE UNINTERRUPTED, ERROR-FREE OR SECURE; (B) THAT

DEFECTS OR ERRORS WILL BE CORRECTED; OR (C) THAT ANY EXECUTIVE WELLNESS BLUEPRINT’S WEBSITE OR THE

SERVERS OR NETWORKS THROUGH WHICH ANY EXECUTIVE WELLNESS BLUEPRINT’S WEBSITE IS MADE AVAILABLE

ARE SECURE OR FREE OF VIRUSES OR OTHER HARMFUL COMPONENTS. APPLICABLE LAW MAY NOT ALLOW THE

EXCLUSION OF IMPLIED WARRANTIES, SO THE ABOVE EXCLUSION MAY NOT APPLY TO YOU.

The Executive Wellness Blueprint websites may include inaccuracies, errors and materials that conflict with these Terms. In the

event of any conflict between anything posted on any Executive Wellness Blueprint website and these Terms, these Terms shall

control.

LIMITATION OF LIABILITY

YOU AGREE THAT IN NO EVENT SHALL EXECUTIVE WELLNESS BLUEPRINT’S LIABILITY TO YOU FOR ANY CLAIM OF ANY

KIND OR DESCRIPTION EXCEED THE AMOUNT OF WHAT YOU PAID TO EXECUTIVE WELLNESS BLUEPRINT PRECEDING

THE DATE IN WHICH THE FACTS GIVING RISE TO A CLAIM AGAINST EXECUTIVE WELLNESS BLUEPRINT OCCURRED.

YOU WAIVE ANY RIGHT TO SPECIAL, INDIRECT OR CONSEQUENTIAL DAMAGES OF ANY KIND OR DESCRIPTION.

IMPOSSIBILITY

FORCE MAJEURE

Executive Wellness Blueprint is hosted on servers maintained by other people, much like almost everyone else with a website. We,

therefore, cannot be and are not responsible to you for any server outages or downtime, any buffering problems, or any delay,

damage, or failure caused by or occasioned by any act of God, act of nature or the elements, terrorism, insurrection, revolution or

civil strife, piracy, civil war or hostile action, labor strikes, acts of public enemies, federal or state laws, rules and regulations of any

governmental authorities having jurisdiction over the premises, inability to procure material, equipment, or necessary labor in the

open market, acute and unusual labor, material, or equipment shortages, or any other causes beyond the control of Executive

Wellness Blueprint, (Collectively, an “Force Majeure Event”). Delays due to any of the above causes shall not be deemed to be a

breach of or failure to perform under this Agreement. Executive Wellness Blueprint shall not be required against its will to provide

refunds or make any adjustments except in accordance with applicable law.

FAILURE TO PERFORM SERVICES

In the event Executive Wellness Blueprint cannot or will not perform its obligations in any or all parts of this Agreement, it (or a

responsible party) will:

● Immediately give Notice to customer via the Notice provisions detailed in this Agreement; and

● Issue a refund or credit based on the customer agreement signed or provided; and

● Excuse customer of any further performance and/or payment obligations in this Agreement.

ASSIGNMENT OF RIGHTS

Executive Wellness Blueprint may assign its rights under this Agreement at any time, without notice to You. Your rights arising under

this Agreement cannot be assigned by without Executive Wellness Blueprint’s or its assigns express written consent, particularly if

you have paid for coaching or classes. For example, you “selling” your username and password or otherwise sharing your password

with others. If we notice tons of IP addresses using the same log in, we expressly reserve the right to cancel or suspend your

access to the materials or content on this Site.

INFORMATION; REGISTRATION; USER NAMES AND PASSWORDS

You are fully responsible for all transactions with, and information conveyed to, Executive Wellness Blueprint under Your Account, if

you have one. You agree to immediately notify Executive Wellness Blueprint of any unauthorized use of Your password or User

name or any other breach of security related to Your account. You agree that Executive Wellness Blueprint is not liable, and You will

hold Executive Wellness Blueprint harmless, for any loss or damage arising from Your failure to comply with any of the foregoing

obligations.

ARBITRATION, GOVERNING LAW, AND ATTORNEYS’ FEES

This Agreement shall be governed, construed, and interpreted in accordance with the laws of the State of Washington without

regard to any choice of law provisions.

Litigation sucks, so you agree that in the event of any arbitration or litigation, each Party will each bear its own costs and attorneys’

fees, regardless of who is deemed the prevailing party. The foregoing notwithstanding, if either You or Executive Wellness Blueprint

commences an action in a court of law or equity and the responding Party successfully moves such court to compel arbitration, the

Party who moved for the order compelling arbitration shall be entitled to recover its reasonable costs and attorneys’ fees incurred on

the motion to compel from the other Party.

CHANGES TO TERMS

Executive Wellness Blueprint reserves the right to change these Terms, in whole or in part, from time to time at Executive Wellness

Blueprint’s sole and absolute discretion, and to provide You with notice of such change by any reasonable means, including without

limitation posting revised Terms.

By Your continued use of Executive Wellness Blueprint’s Site, unless You expressly terminate this Agreement in writing to

SEVERABILITY

If any Term set forth herein is deemed unenforceable under any applicable law, the remaining Terms shall remain in full force and

effect and these Terms shall be read, collectively, as if the unenforceable Term(s) were omitted.

WAIVER

No waiver by Executive Wellness Blueprint of any breach or default of these Terms will be deemed to be a waiver of any preceding

or subsequent breach or default.

HEADING

Any heading, caption or section title contained herein is inserted only as a matter of convenience, and in no way defines or explains

any section or provision hereof.

Any notice required to be given to Executive Wellness Blueprint under or related to these Terms must be in writing to:
